LBH is committed to making access to psychiatric and telepsychiatry services straightforward and efficient. Our referral process ensures that individuals seeking mental health care, whether through self-referral or professional referral, can quickly connect with our team for evaluation and treatment.

Self-Referral

Who Can Self-Refer: Individuals seeking psychiatric services for themselves can directly contact LBH without a formal referral. 

Process:
1. Contact LBH via phone at (508) 258-4801, email at care@lampstandbh.com, or through the contact form on www.lbhservices.org.
2. Provide basic information (name, contact details, and a brief description of needs or symptoms).
3. Schedule an initial consultation (in-person or telehealth) with a licensed psychiatrist or nurse practitioner. 

Next Steps: During the initial consultation, a comprehensive assessment will be conducted to develop a personalized treatment plan.

Professional Referral

Who Can Refer: Primary care physicians, therapists, counselors, social workers, or other healthcare providers can refer patients to LBH.

Process: 1. The referring provider submits a referral form (available at www.lbhservices.org/referral) or contacts LBH directly via phone at (508) 258-4801 or email at care@lampstandbh.com.
2. Include relevant patient information, such as:
■ Patient’s name, contact information, and insurance details (if applicable).
■ Reason for referral (e.g., specific mental health concerns, need for medication management).
■ Relevant medical or psychiatric history, if available.

3. LBH staff will contact the patient within 48 hours to schedule an initial consultation.

Next Steps: The patient will undergo an evaluation, and LBH will communicate with the referring provider to ensure coordinated care, if requested.

Community or Organizational Referral

Who Can Refer: Community organizations, schools, or social service agencies can refer individuals to LBH for mental health support.

Process

2. Provide a brief overview of the individual’s needs and any relevant background information.
3. LBH will reach out to the individual (or their guardian, if applicable) to arrange an appointment.
